Independence Waiver program Eligibility
Pa residents age 18 to 59 with a severe
physical disability requiring a nursing facility
level of care. The disability must result in
substantial functional limitations in three or
more of the following major life activities:
mobility, communication, self-care,
self-direction, capacity for independent living,
and learning.

Attendant
Care Program Eligibility
Pa residents ages 18 through 59 who are mentally
alert and have physical disabilities.
The purpose of the Pa. Attendant Care
Program is to enable individuals with
physical disabilities to perform
activities of daily living.

Attendant
Care Act 150 Eligibility
- Be at least 18 years of age but less than 60
years of age
- Be mentally alert and capable of:
*Managing one’s own financial affairs
*Managing one’s own legal affairs
- Experience any medically determinable
physical impairment that can be expected to last
for a Continuous period of not less than 12
months or that may result in death.
- Due to physical impairment, require
assistance to complete functions of daily
living, self-care and mobility.
- Be a citizen of the United States and a
resident of Pennsylvania.

The Pennsylvania Independent
Enrollment Broker
The Office of Long-Term Living
launched the PA Independent Enrollment
Broker, effective December 1, 2010. The
Independent Enrollment Broker is a
contracted statewide entity to
facilitate and streamline the
eligibility/enrollment process for
applicants seeking services for several
Pennsylvania waivers/programs.
The Independent Enrollment Broker
provides enrollment services for
applicants with physical disabilities
who are 18-59 years of age applying for
Attendant Care, COMMCARE, Independence,
OBRA and the 0192 (AIDS) Waivers and the
Act 150 Attendant Care Program. Area
Agencies on Aging (AAA) provide
eligibility/enrollment services for
applicants over age 60.
The PA independent Enrollment Broker
will work in close collaboration with
service coordination providers and key
stakeholders to respond to needs,
address issues, and ensure participants
receive prompt, high quality service.
